204 - Incident and Injury Reporting

  1. Employees shall report to their supervisor any job-related injury/illness immediately following an event or as soon as it becomes apparent that an injury/illness has occurred, regardless of how slight. Employees must also report job-related incidents in which no signs of injury are immediately present, but indications exist that an injury may develop or be aggravated by additional work.
  2. If your supervisor is not available, the injury shall be reported to available management.
  3. The supervisor shall immediately contact the site on-call Medical and FFD Services staff.
  4. An Employee Injury/Illness Report and a CR shall be completed. Reports shall be provided to Medical and FFD Services normally within 24 hours.
  5. If a situation arises and you need medical care for a work-related medical problem during your off-duty hours, notify your supervisor or the Operations shift manager who will notify the Medical and FFD Services staff so that arrangements can be made for you to see one of the doctors listed on the panel of physicians.
  6. In an emergency, the doctor providing treatment does not have to be a company-approved physician. After the period of the emergency, you will be referred to a company-approved physician.
  7. Employees who have experienced an electric shock, been injured by an airless sprayer, high pressure liquid injection, received puncture wounds, been bitten by an animal, inhaled toxic gas, or received an eye injury shall be evaluated by a doctor unless directed otherwise by Medical and FFD Services staff.
